    目的 探讨参附注射液对大鼠心肌缺血再灌注期间的心电图演变过程的影响及作用机制。 方法 取 50 只大鼠,按体重排序随机分为 5 组,各 10 只,除假手术组外均采用冠状动脉左前降支结扎法建立缺血再灌注模型;假手术组:左冠状动脉前降支穿线、不接扎,穿线前 10 min 股静脉推注 2 mL/ 100 g 注射用生理盐水;模型组: 缺血前 10 min 股静脉推注 2 mL/ 100 g 注射用生理盐水;参附低、中、高剂量组分别在缺血前 10 min 股静脉推注 1、 2、4 mL/ 100 g 参附注射液。灌注结束后取心肌组织,检测心肌细胞凋亡情况及心肌组织中蛋白激酶 B(Akt)、磷酸化 Akt(p-Akt)、糖原合成酶激酶-3β(GSK-3β)、磷酸化 GSK-3β(p-GSK-3β)、哺乳动物雷帕霉素靶蛋白(mTOR)、磷酸化 mTOR(p-mTOR)蛋白表达情况。 结果 与假手术组比较,模型组缺血后 10 min、30 min 及再灌注 10 min、90 min 心电图 ST 段明显抬高(P <0. 05);与模型组比较,参附低、中、高剂量组缺血后 10 min、30 min 及再灌注 10 min、 90 min 心电图 ST 段明显降低(P <0. 05);与参附低、中剂量组比较,参附高剂量组缺血后 10 min、30 min 及再灌注 10 min、90 min 心电图 ST 段明显降低(P <0. 05);参附低剂量组与中剂量组比无明显差异(P >0. 05)。 缺血后30 min 参附中、高剂量组 VT、VF 发生率均低于模型组(P <0. 01);再灌注 90 min 参附高剂量组 VT、VF 发生率均低于模型 组(P <0. 01)。 与假手术组比较,模型组心肌细胞凋亡指数、p-Akt、p-GSK-3β、p-mTOR 蛋白相对表达量均升高(P < 0. 001);与模型组比较,参附低、中、高剂量组心肌细胞凋亡指数、p-Akt、p-GSK-3β、p-mTOR 蛋白相对表达量均降低 (P <0. 001);与参附低剂量组比较,参附中、高剂量组心肌细胞凋亡指数、p-Akt、p-GSK-3β、p-mTOR 蛋白相对表达 量均降低(P <0. 05),且参附高剂量组低于参附中剂量组(P <0. 05)。 结论 参附注射液可有效改善心肌缺血再灌 注损伤心电图变化,降低心律失常发生率,抑制心肌细胞凋亡,可能与抑制 Akt / GSK-3β 信号通路激活有关。

    Abstract:

    Objective To investigate the effect and mechanism of Shenfu injection on the evolution of electrocardiograms during myocardial ischemia-reperfusion injury in rats. Methods Fifty rats were randomly divided into five groups (10 rats in each group) according to body weight reordering. In the sham operation group, the left anterior descending coronary artery was punctured without ligation, and 2 mL/ 100 g saline was injected into the femoral vein at 10 minutes before puncturing. In the model group, 10 minutes before ischemia, 2 mL/ 100 g saline was injected into the femoral vein. In low, middle, and high dose groups, 1, 2 and 4 mL/ 100 g Shenfu injection was applied to the femoral vein at 10 minutes before ischemia. At the end of perfusion, cardiomyocyte apoptosis was detected and protein expressions of protein kinase B (Akt), phosphorylated Akt (p-Akt), glycogen synthase kinase-3β (GSK-3β), phosphorylated GSK-3β (p-GSK-3β), mammalian rapamycin target protein ( mTOR) and phosphorylated mTOR ( p-mTOR) were detected. Results Compared with the sham operation group, the ST segment of the ECG in the model group was increased significantly (P <0. 05). Compared with the model group, the ST segment of the ECG in Shenfu low, middle and high dose groups was decreased significantly (P <0. 05). Compared with Shenfu low and middle dose groups, the ST segment of the ECG in the Shenfu high dose group was decreased significantly (P < 0. 05). No significant difference was observed between the two groups (P >0. 05). The incidence of VT and VF in middle and high dose groups was lower than that in the model group (P <0. 01). The incidence of VT and VF in the high dose group was lower than that in the model group (P <0. 01). Compared with the sham operation group, the apoptosis index and relative expression of p-Akt, p-GSK-3β and p-mTOR in the model group were increased (P <0. 001). Compared with the model group, the apoptosis index and relative expression of p-Akt, p-GSK-3β and p-mTOR in Shenfu low, middle, and high dose groups were decreased (P <0. 001). Compared with the low dose group, the apoptosis index and relative expression of p-Akt, p-GSK-3β and p-mTOR in Shenfu middle and high dose groups were decreased (P <0. 05), and those in the high dose group were lower than those ub the middle dose group (P <0. 05). Conclusions Shenfu injection effectively improves the electrocardiogram changes of myocardial ischemia-reperfusion injury, reduces the incidence of arrhythmia, and inhibits myocardial cell apoptosis, which may be related to inhibiting activation of the Akt / GSK-3β signaling pathway.
